NEW ORLEANS – LCMC HealthCare Partners, LLC and Humana Inc.(NYSE: HUM), one of the nation’s leading health and well-being companies, have entered into a value-based agreement designed to offer a coordinated, personalized experience to help Humana Medicare Advantage members achieve their best health. Details were shared in a press release.
The value-based contract expands upon Humana’s and LCMC Health System’s existing Medicare network contract, which will continue to provide Humana Medicare Advantage members in Greater New Orleans with access to LCMC facilities throughout Greater New Orleans.
The expanded agreement provides in-network access for Humana Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) health plan members across New Orleans seeking care at LCMC’s newly formed clinically integrated network, LHP.
LHP is a physician-led organization, focused on delivery of value to patients, providers and payers through a population health model of care. The LHP network is comprised of over 900 providers, including physicians employed by LCMC Health, community physicians, LCMC’s five member hospitals and post-acute specialists.

The value-based agreement with LCMC HealthCare Partners and Humana is in keeping with Humana’s longstanding commitment to value-based care, which emphasizes:
- More personal time with health professionals and personalized care that is tailored to each person’s unique health situation;
- Access to proactive health screenings and programs that are focused on preventing illness;
- Improved care for people living with chronic conditions with a focus on avoiding health complications;
- Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
- Reimbursement to physicians linked to the health outcomes of their patients rather than based solely on the quantity of services they provide (fee-for-service).
Humana has an extensive and growing value-based care presence. As of June 30, 2019, Humana has approximately 2.3 million individual Medicare Advantage members and approximately 115,000 commercialmembers who are cared for by more than 58,000 primary care physicians in more than 1,000 value-based relationships across 43 states and Puerto Rico.
